New Patient Fear Free Survey

As a Fear Free Certified Professional team, we want to reduce the fear and stress our patients might experience before, during or after their visit with us. To better serve you, it is important for us to understand what might be the cause(s) for their anxiety. This information will help us to cater our care to their individual experience and needs. Please answer the following questions to the best of your ability so we can take into consideration both you and your four-legged family members' preferences.

Species
Are they reluctant to get in the car?
On the way to vet visits, have you seen them show any of the following?
Do they have a preference in care givers? (if they cause fear or if they put at ease)
Have they shown dislike or avoidance of any of the following?
Are they allowed to have treats?

If they do respond positively to treats being offered at the vet, for best results or for patients with stomach-sensitivity or allergies, we highly recommend bringing their favorite, familiar treats with you to each visit. If they don't have dietary or medical restrictions regarding treats, we do routinely offer squeeze cheese, peanut butter and digestive treats to dogs and catnip treats to cats because they are popular and well-tolerated.

Please let us know if you have allergies among your family so we can exclude any of these treats for your own safety.

Additionally, if they are more at ease with the presence of a toy, ball or blanket - any portable, prized possession that might comfort them during their visit, you are certainly welcome to bring it with you.
